Dr. Dempsey will present a case examining the intersection of mold exposure, mast cell activation syndrome (MCAS), and neuroinflammation, while Dr. O’Hara will review a pediatric case of COVID-induced PANS/Basal Ganglia Encephalitis (BGE) and the potential for misdiagnosis as a neurodevelopmental or psychiatric disorder. Following the case presentations we’ll open up the discussion for a live Q&A session, providing an opportunity for attendees to ask questions and further explore the clinical considerations raised by each case.
Case #1 - A Case of Mold- Induced Neuropsychiatric Symptoms
Dr. Dempsey will review a case of a post-menopausal female with a history of vector-borne infections and MCAS who suddenly went into a major flare associated with severe anxiety, OCD-like behavior, and depression. The case will be reviewed from the perspective of identifying triggers, determining diagnostics and then reviewing her approach to treatment. Participants will learn about the intersection between Mold, MCAS and Neuroinflammation.
Case #2 - Regressive ASD or COVID induced Basal Ganglia Encephalitis (BGE): You Be The Judge
PANS & PANDAS or Autoimmune Encephalitis of the Basal Ganglia are complex autoimmune diseases that require a multi-system approach. PANDAS (Pediatric Autoimmune Neuropsychiatric Disorder Associated with Strep) involves antibodies from a strep infection reacting with brain tissue (specifically the basal ganglia in the brain) and triggering an abrupt onset of immune dysregulation and movement, learning and behavioral problems including OCD, anxiety and tics. With PANS (Pediatric Acute-Onset Neuropsychiatric Syndrome), other infectious etiologies besides strep (yeast, viruses including COVID, other bacteria, parasites), toxic exposures, and metabolic abnormalities are among other potential triggers for the immune dysregulation resulting in an abnormal autoimmune reaction and negative behavior, physical, and cognitive changes. Dr. O’Hara will discuss how some children are misdiagnosed with ASD or OCD or other neurodevelopmental or neuropsychiatric disorders when the assessment and treatment of PANS/PANDAS/BGE should be considered. Dr O’Hara will demonstrate this through an illustrative case study of COVID-induced PANS. Participants will learn the research-based functional medicine approach to the appropriate diagnosis of these children.

Tania Dempsey, MD, ABHIM is a nationally recognized expert in complex, multisystem diseases, with expertise in mast cell activation syndrome (MCAS), mold and environmental toxins, Lyme disease, and vector-borne illnesses. Board-certified in Internal Medicine and Integrative and Holistic Medicine, she earned her medical degree from Johns Hopkins University and completed her residency at NYU Medical Center. Dr. Dempsey is the founder of AIM Center for Personalized Medicine, serves as Secretary on the ILADS Board of Directors, and hosts the Mast Cell Matters podcast.
Nancy O’Hara, MD, MPH, FAAP is a board-certified pediatrician and internationally recognized leader in integrative and holistic care for children with chronic illness and neurodevelopmental and neuropsychiatric disorders. Her clinical and educational work focuses on PANS/PANDAS, tick-borne diseases, mold and mycotoxin illness, post-infectious syndromes, and folate receptor autoantibodies (FRAAs). Dr. O’Hara is the author of Demystifying PANS/PANDAS and has published research on FRAAs and PANS/PANDAS. She also trains and mentors clinicians worldwide and co-leads an integrative Pediatric Fellowship through Psychiatry Redefined. She serves on the ILADS Board of Directors, the Medical Education Committee, as well as the Pediatrics Committee.
